Individual who submits all the data required for Prior Notice. The submitter of prior notice information, regardless of the method of or person transmitting the information, is responsible for the accuracy of that information. Usually this is the shipper or manufacturer. |
| Transmitter (Lines 4 and 5) |
The transmitter transmits data provided by the submitter exercising diligence and care to transmit the information provided by the submitter accurately. |
| Shipper Registration # (Line 6) |
Required if the shipper manufactures, processes, packs or hold foods. |
| Shipper (Line 7) |
Name and address of the owner or exporter of the article of food who consigns and ships the article from a foreign country or the person who sends an article of food. |

The name and address of the owner if different from the importer or ultimate consignee. If the registration number is provided, city and country may be provided instead of the full address. |
| Country Shipped (Line 9) |
The country in which the article of food is loaded onto the conveyance that brings it to the US. |

Consumption entries 01, warehouse entries 21, and Temporary Importation Bond Entries 23, and other codes currently provided to CBP at entry. There will also be codes for IT, T&E, mail, FTZ etc. |

The water, air or land port at which the article of food is imported or offered for import into the US, i.e., the port where the article of food first arrives in the US. |

If a registration number is provided, city and country may be provided instead of the full address. The identity of the ultimate consignee is not required for an article of food that is imported or offered for import for transshipment through the US under a T&E entry. |

If thee codes are not applicable, the carrier’s name and country must be submitted. |

Person who engages in growing and harvesting or collection corps (including botanicals), raising animals (including fish, which includes seafood) or both. Used for food arriving in its natural state. |
| Country of Production (Lines 33) |
The FDA Country of Production is not always the same as the CBP Country of Origin. For an article of food in its natural state, where the food was grown, harvested or collected and readied for shipment to the US. If the article in no longer in its natural state, the country were the article of food was processed or canned. |
